Students Identity Card Data Form

​Kindly follow the below given instructions while filling the Form.

1. Select your child's Class, Division and Roll No. and click on search.

2. The Name of your child will be displayed, if the name is of your child, click on YES, else click NO.

3.  Enter the Address, Father's Mobile No., Mother's Mobile No. and the Blood Group of the Child and click on Submit.

4.  Kindly check all details before submitting the Form.  No changes shall be made once the Form is submitted.

5   The Form can be filled only once.
